Direct posterior composite restorations, are there any limits?
Placement of direct posterior composite restorations is core business of dental practitioners. Om social media it seems that the gold standard are perfectly placed small sized restorations with coloured fissures, however, often placed in doubtfull indications. But what about large defects on endo treated teeth?, all cusps lost? Subgingival margins? Do we need to go indirect then or can we still succesfully place direct restorations? Niek Opdam will show that there are almost no limits in placement of posterior composites, but at the same time will adress the risk factors determining succes.
In this lecture a list of topics will be discussed:
- Choice of materials and adhesives to place reliable posterior composites
- Factors determining restoration longevity
- What is more important: longevity of the restoration or longevity of the tooth?
- Direct composites for large and/or complex posterior defects
- Complex direct posterior composites: tips and tricks regarding matrix applience, composite application, contact points, finishing etc.

Niek Opdam
Niek Opdam graduated in 1980 and is retired associate professor at the Radboud University Medical Centre at Nijmegen, The Netherlands. He has lectured throughout the world on restorative dentistry, practice based research and treatment of Tooth wear. In 1998 he received his PhD on a Thesis on posterior composite. He has now published more than 150 papers in peer-reviewed journals and his H-Index is 47 on Web of Science. In 2017 he received the Ryge-Mahler award from the Dental Materials Group of the IADR for his clinical research and in 2021 the Ivar Mjör award for practice based research. From 2012-2015 he was Visiting Professor at the Federal University of Pelotas, Brazil. Niek Opdam was president of the practice-based research Group of the IADR and president of the European Federation of Conservative Dentistry EFCD. Niek Opdam has a part-time referral practice with differentiation in adhesive dentistry in Den Bosch, The Netherlands.
